The connect app registers your extension account via cloud servers, so your PBX must be accesible to those servers for registration purposes. From the other extension registrations in your screenshot it seems you are using the PBX locally to them, so if your SIP port is not accessible to the app servers it will never be able to register. At this moment it seems like a firewall issue.
Can you share the ports that should be forwarded for Vital PBX Connect to register properly. I currently have the below forwarded to the server IP.
