Transfer calls seamless from desktop to webrtc or vitalpbx softphone

Is there a way top seamlessly while on a call transfer or switch a device? example, I’m using a polycom / grandstream deskphone and need to move to my cell either with webrtc or the vitalpbx app without the caller noticing it?

The described behavior isn’t supported by Asterisk.
