Random PBX Inbound Calls Failing

Randomly throughout the week on non-specific days and at non-specific times the PBX will announce to callers “I’m sorry you have dialed the wrong number. Please check the number and try again”. I have to reboot the entire PBX to get it to stop the behavior. It will not allow any inbound calls to any DID configured in the PBX when this happens.

We have had this PBX running for years through multiple upgrades and is currently on the most stable version 4.1.0-7 and Debian 11 5.10.0-32-amd64 on a cloud server with and AMD EPYC-Milan Processor 8 gigs of RAM. It handles about 500-1000 inbound calls a day and 250 or so outbound.

Has anyone else experienced this and found a fix? I’m tempted to rebuild the entire PBX from scratch with new settings (not imported) to fix the issue but I’d like to see if anyone else has experienced this before I go nuclear on it.

You would really have to get some logs of when the issue is happening for us to better help you, otherwise there is no real way to tell what could be causing the issue. If it only happens at time, I wouldn’t think its config issue otherwise all your calls would fail, it would seem like to much load at time could cause an issue like this. Again hard to tell without logs from your side.
