It is hosted here at home, behind an opnsense firewall with static IP.
I set the Media and Signal address’ to my external ip.
An interesting turn of events …
I changed the profile on the trunk to 3cx.
All started working 
Any idea why that may be?

Oh, I got it. You created a new PJSIP Profile named 3CX.
If i did, i dont remember doing it lol
The difference between the 2 profiles ( default and 3cx - both pjsip ) is as below:
Any idea why the 3cx one works and the default does not. It would be nice to know going forward.
You can start disabling/enabling option by option all those parameters that are configured differently in the 3CX profile.
